Who we are
ActiveLife Chiropractic runs chiropractic clinics at Wisma Atria (435 Orchard Road, #10-01) and Novena Specialist Center (8 Sinaran Drive, #07-01), Singapore. This policy explains how we collect, use and protect your personal data, in line with Singapore’s Personal Data Protection Act 2012 (PDPA).
What we collect
When you contact us: your name, phone number, email address, preferred clinic and appointment times, and anything you choose to tell us when you call, WhatsApp or email us.
When you become a patient: the information needed to assess and treat you, such as your medical history, symptoms, examination findings, X-rays or scans, treatment notes and payment details.
When you use this website: basic, anonymous usage information such as pages visited, device type and approximate location (see “Cookies and analytics” below).
How our website handles your details
Our booking request and the WhatsApp buttons open WhatsApp on your own device with a message ready for you to send. Nothing is sent to us until you press send in WhatsApp.
Our online new-patient health form does not store anything on our website. Your answers stay in your browser until you save or print the form yourself and bring or send it to us.
Why we use your data
We use your personal data to reply to your enquiries, arrange and manage appointments, assess and treat you, keep proper clinical records, process payments and insurance or corporate claims, send appointment reminders, and meet our legal and regulatory obligations.
We will only send you health tips, offers or newsletters if you have agreed to receive them, and you can opt out at any time.
Who we share it with
We do not sell your personal data. We share it only where needed to care for you or run our clinics, for example with doctors or specialists you are referred to (with your consent), insurers or your employer for claims you ask us to submit, imaging centres, and service providers who support our clinic systems. These parties must protect your data and use it only for the purpose we give them. We may also disclose data where the law requires it.
Cookies and analytics
This website uses Google Analytics to understand how many people visit, which pages are useful and which buttons are tapped (for example “WhatsApp” or “Call”). This uses cookies and does not tell us who you are. Our pages also show embedded Google Maps. You can block or delete cookies in your browser settings, and the site will still work.
How we protect and keep your data
We take reasonable security measures to protect your personal data against unauthorised access, use or loss. We keep clinical records for as long as required by Singapore law and professional guidelines, and other personal data only for as long as it is needed for the purposes above.
Your rights
You may ask to see the personal data we hold about you, ask us to correct it, or withdraw your consent to how we use it. If you withdraw consent, we will explain any effect this may have on our ability to care for you. We will respond within a reasonable time, normally within 30 days.
